During the school year:

 Parent/Guardian, please bring:

• Valid ID of authorized Parent/Guardian

• Chromebook/ laptop

• Textbooks

• Library materials

• Uniforms: Athletic/Band/Choir/Orchestra

• Name and address of new school

• Remaining Cafeteria Fees

When withdrawing during the school year, out-of-state schools may request student records by fax to 972-727-0345, Attention Dawn Blue.

Record Requests should be initiated by the receiving school.

Texas schools should initiate student records requests to Curtis Middle School through Texas Records Exchange, also known as T-REx.

During the summer:

Parent/Guardian will complete a  Summer Withdrawal Form  and email it back to the enrollment office at  dawn.blue@allenisd.org

All documents needed to enroll at your student's new school such as transcripts, report cards, and immunizations are attainable in Skyward for you to download prior to withdrawal.
